Customer Special Design Order Form

Please read carefully.

Customer special design orders must be accompanied with a 50% deposit (unfortunately, customers have ordered special pieces
and left us "high and dry" by not purchasing them after completion. This causes us a lost of time and profit.) of price quoted by
designer.

After completion, a picture of the jewelry will be emailed (or sent) to the customer for final approval of design. When designed is
approved by customer, he/she pays the remaining 50%.

The item(s) will be shipped out within 24hrs of receipt of payment. Shipping is Free on domestic orders: however, appropriate sales
tax for Pennsylvania residents apply.

For international customers, shipping and handling charges will be quoted at time of order.

There are no returns on Customer Designed pieces. We will strive to design your piece as specified until you are satisfied!

All customer designed items must be paid in full within 7 days of email notification of completion. No deposit may be returned
after 24hrs of submission of original order form.
